OneCard Refund Claim Form

The OneCard Office will honor the following claims:

Patrons may request a refund for lost or bad printouts/copies that result from mechanical or network failure or equipment defect. The OneCard Office will not reimburse patrons for misprints resulting from user error, or for paper jams resulting from misuse of the printer/copier.

Accepted claims will be refunded to the patron's OneCard within 48 business hours.
